Enhancing remote home monitoring for adults living with stroke-related disability to support personalized rehabilitation and healthy aging

Stroke is a leading cause of long-term disability, and despite advances in rehabilitation, many individuals experience functional decline after discharge due to limited access to ongoing, personalized rehabilitation when needed. Current models rely on episodic, clinic-based assessments that do not reflect real-world functioning and do not allow long-term follow-up over years and decades.
Through focus groups and a community engagement activity involving stroke survivors, caregivers, health professionals, and Health Authority representatives we will increase our understanding of the diverse priorities and needs regarding real-world monitoring at home to support proactive, personalized rehabilitation and healthy aging in stroke survivors. We will further discuss the relevance of contextualizing functioning and health data collected through remote monitoring in the home environment.
We will host two virtual meetings and one hybrid workshop (in-person, with a virtual option for those who cannot attend in person to reduce geographical and mobility barriers) for stakeholders. In the hybrid workshop, a hands-on demonstration of how a digital monitoring system might look provided by a mobile health development platform partner (Pathverse), along with small-group discussions, will help shape possible directions and identify preliminary opportunities and barriers, and equity, accessibility, and usability considerations. Through this engagement, we aim to strengthen the collaboration between stakeholders.
Participants from different backgrounds will be included to ensure a multiperspective approach, with efforts made to ensure diversity and representation across different groups, including age, sex, geography, racial and ethnic background: stroke survivors (n=10), caregivers (n=10), physicians (n=2), allied health professionals (n=8), and representatives of the Health Authority (n=2). This will help identify gaps, challenges, and opportunities to enhance remote monitoring of real-world functioning and healthy living among stroke survivors in their home environments.
Findings will inform the future development of research, innovation, and implementation of remote home monitoring to support individualized rehabilitation and enhance functioning and healthy aging over time in stroke survivors.
